
The 2005 overall winner was Hospice of the Valleys. Based in Tredegar, they provide a specialist round the clock, 365 days a year palliative care service for people living in South Wales. The team includes a doctor, specialist nurses, social workers and therapists to provide free high quality specialist care for people at home. They give comprehensive support for their families from diagnosis, through to treatment and if appropriate into terminal care and bereavement.

4Sight - Bognor Regis - offers a high quality service supporting blind and partially sighted people, and their carers, in West Sussex.
Coping with Cancer North East - Newcastle-Upon-Tyne - offers emotional and practical support for people with cancer and their carers across Tyneside.
Footprints Women’s Centre – Belfast - working to build confidence and encourage healthier lifestyles for women and their families in the West Belfast region.
InterAct Reading Service – London - helping the recovery and rehabilitation of stroke sufferers through a live reading service provided by professional actors.
Lost Chord – Yorkshire - bringing music therapy concerts to dementia sufferers in residential homes and day centres throughout South Yorkshire.
National Rheumatoid Arthritis – Maidenhead - supports people with Rheumatoid Arthritis and their families to gain equal access to the best standards of care and treatment wherever they live in the UK.
Penhaligon’s Friends, based in Cornwall, is a specialist bereavement support service for Cornish children and their carers.
The Positive Place -South East London - working to meet the needs of people with HIV in South London.
Women’s Health & Family Services – London - helping to preserve and protect the health of ethnic minority women and their families in London’s Tower Hamlets.
